Preparing for the Season
Sports physicals help determine whether an athlete can safely participate in organized activities. The evaluation may include a review of health history, vital signs, heart and lung assessment, vision screening, joint movement, strength, and flexibility as appropriate.
Completion of an examination does not guarantee clearance. Additional records, testing, or follow-up may be recommended based on the evaluation.
What to Bring
Please complete the patient and family-history portions of your required form before arriving. Additional evaluation or documentation may be necessary depending on the athlete’s health history.
